Please bring the following documents when registering a new student:
Copy of child's birth certificate
Copy of parent/guardian photo ID (see below examples):
Driver's license, passport, military ID, or green card
Copy of guardianship document (if applicable)
Copy of custody agreement and/or any other relevant legal documentation (if applicable)
Copy of immunization records (can be provided in native language). Click this link for current immunization requirements. Physical and immunization records can also be sent from your pediatrician to the Health office for your student's school
Other Required Documents
Proof of residency (one of the following items):
Copy of mortgage payment within 60 days in parent/legal guardian's name
Copy of tax bill in parent/legal guardian's name (property or excise)
Copy of lease in parent/legal guardian's name & rental payment within 60 days
Include a copy of the SIGNED mortgage/lease of the Dennis or Yarmouth resident living at the provided address
IMPORTANT: Must be resubmitted annually by July 30th
Proof of occupancy (one of the following items):
Copy of utility bill within 60 days (see below examples):
Gas, electric, oil, water, sewer, home telephone (not a cell), or cable
NOTE: Utility companies can provide a set-up notice if property is newly acquired
Copy of payroll stub within 60 days
Copy of bank or credit card statement within 60 days
Copy of current vehicle registration
Copy of insurance bill or payment within 60 days (see below examples):
Home, rental, or health
Copy of letter from approved agency (see below examples):
Social security, WIC, SNAP, or disability information
IMPORTANT: If the DYRSD Residency Affidavit was used for the proof of residency, then one of the above items must also be resubmitted annually by July 30th
Copy of physical within the last 12 months (or a promise to schedule an appointment)
