Required Documents

Required Documents for Registration

Please bring the following documents when registering a new student:

  • Copy of child's birth certificate
  • Copy of parent/guardian photo ID (see below examples):
    • Driver's license, passport, military ID, or green card
  • Copy of immunization records (can be provided in native language).  View current immunization requirements. Physical and immunization records can also be sent from your pediatrician to the Health office for your student's school
  • Copy of physical within the last 12 months (or a promise to schedule an appointment)

Proof of residency (provide TWO DOCUMENTS; each document must be from a different category below): 

Category A: Lease/Mortgage/Town-issued Documents

  • Copy of current signed lease in parent’s/legal guardian's name
  • Copy of mortgage statement within 60 days in parent’s/legal guardian's name
  • Property deed
  • Property or excise tax statement for current year in parent’s/legal guardian's name
  • Section 8 agreement
  • DYRSD Residency Affidavit (if applicable)
    • Must be notarized unless all people are present for the registration
    • Must include one of the residency documents mentioned above in the homeowner’s/renter’s name

Category B: Massachussetss RMV-Issued Documents

  • RMV-issued correspondence within past 60 days
  • Current vehicle registration with current address

Category C: Bills

  • The following items will be accepted if they are dated within the past 60 days and contain the current address:
    • Utility bill, including setup notices for new residents (Electric, landline telephone, water, sewer, cable, satellite, heating)
    • Credit card
    • Cell phone

Category D: Insurance-related Documents

  • The following items will be accepted if they cover the current year and contain the current address:
    • Auto insurance policy
    • Renter’s insurance policy
    • Homeowner’s insurance policy

Category E: State/Federal/Municipal/County-Issued Documents

  • The following items will be accepted if they are dated within the past 60 days and contain the current address:
    • Medicaid statement
    • Jury duty summons
    • Court correspondence
    • Letter from approved government agency (Department of Children and Family Services, Department of Transitional Assistance, etc.)

Category F: Financial-Related Documents

  • The following items will be accepted if they are dated within the past 60 days and contain the current address:
    • Payroll stub
    • Bank statement
    • Pension/retirement statement
    • SSA statement
    • Auto loan statement
  • W-2 for current year containing the current address
