Dennis-Yarmouth School Students; Homeschooled Students, Private School Students; and Student Summer Residents:
 

Employment Permit Application forms (promise of employment forms) can be downloaded here. Read, print, and complete the entire application (14-15 year olds must have a Doctor’s signature).

Bring completed applications to the Dennis-Yarmouth High School Main Office.

School Year Office Hours: available Monday-Friday 7:00 am - 2:50 pm

Summer Office Hours: Monday-Friday 7:00 am - 1:00 pm 

Non Dennis-Yarmouth High School Students, need to bring the following information:

• proof of school currently attending (school ID, transcript, report card, etc.)
• proof of age (license, passport, birth certificate)
• local address where you are staying during employment

Lastly, we will input the information into the computer and print the Employment Permit which will require the student’s signature (signed in the presence of the issuing agent). The student will take the permit to the employer.
