Due to recent changes under the Paperwork Reduction Act, (President Biden signed into Law on December 23, 2024) we are no longer automatically mailing ACA 1095-C forms. Your Health Insurance company will send you a 1095 – B. If you need a copy of your 1095 – C form for tax purposes, please submit a request to Paula Kiser and we will provide it to you within 30 days.
