Sports Sign Up Instructions

  1.  To Sign up for a sport at Mattacheese you must do so through FamilyID.com
  2. Online Payment Now Available: Pay Online
  3. You must have a valid Physical. Physicals are good for 13 months and must be checked by the Athletic Director and School Nurse.

Registration is done completely online through the company Family ID. All parents will need to register through this online web page. You must registar before you are able to participate.

Mattacheese Middle School is excited to announce that we are now offering the convenience of online registration through the partner company, FamilyID. FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for our programs, and helps us to be more efficient and environmentally responsible. When you register through FamilyID, the system tracks your information in your FamilyID profile, so you enter your information only once for multiple uses, multiple family members and multiple programs.

This year registration will be done online through Family ID. All parents/guardians will need to register through the online web page. You must register your student within this time period for them to participate in Sports.

Steps you Need to Take Prior to Registration

  1. Physical Exams: A physical exam must be current (within the past 13 months) and signed by the physician. If the student’s physical expires during the season, they will not be able to play until a new exam is received. Free physical exams will be available by our school physician Dr. Colmer. Please sign up with the nurse if you need an updated physical.
  2. Complete Online concussion Course: State law requires that student/athletes, along with their parent(s)/guardian(s) complete an online concussion course. During registration you will be asked to sign off that you have completed this course: It is available here.
  3. Athletic User Fees: There is a user fee for all of our student athletes. The fee is $50. This year we are excited to announce that we are offering both online payments through UniPay directly on our website as well as payment by check. The link can be found on the left hand side under Online Payment Center on our Athletics page.

If you choose not to pay online a check can be written to Dennis-Yarmouth Regional School District and turned into the Student Athlete’s Coach.

In hopes to never hold back any of our student athletes due to financial reasons it has been our practice that if your student athlete is receiving free/reduced lunch he/she will receive a waiver. We will need documentation for this waiver to be created. If there is an individual issue and you are unable to afford this user fee please email or call me at school so we can discuss each individual case. There is a $100 family user fee limit per school year per building.

We ask that you hold off on payment until your child has officially made a team. Since all of our winter sports are cut programs we want to make sure we don't have to refund any families that pay prior to making a team.

Information Needed to Register

It will be helpful to have the following information on hand to allow for accurate completion of your online registration.

  • Doctor information, health Insurance information, emergency contact information

Registration Process

Follow these steps:

  1. To find your program, click on the link provided by the organization above and select the registration form under the word Programs.
  2. Next click on the green Register Now button and scroll, if necessary, to the Sign Up/Log In green buttons. If this is your first time using FamilyID, click Sign Up. Click Log In, if you already have a FamilyID account.
  3. Sign Up for your secure FamilyID account by entering the account owner First and Last names (parent/guardian), email address and password. select the agreement to the FamilyID Terms of Service. Click Sign Up.
  4. You will receive an email with a link to activate your new account. (If you don’t see the email, check your email filters (spam, junk, etc.).
  5. Click on the link in your activation email, which will log you into FamilyID.com
  6. Once in the registration form, complete the information requested. All fields with a red* are required to have an answer.
  7. Click the Save & Continue button when your form is complete.
  8. Review your registration summary.
